Completing The FormDo I have to submit a Contract for Analysis when I submit samples to AAIPharma? Yes. This form is the document that provides your detailed instructions on what testing you want performed, how the samples should be named and what payment arrangements should be made. It is our roadmap that ensures that we understand your requirements and, in turn, deliver against those requirements to your expectations. This document also contains our Terms and Conditions; important information for both parties as we maintain a good working relationship. Why can't I use our company "job ticket" instead of your Contract for Analytical Services? While your job ticket may be excellent, it may not contain all the information we need to identify your samples, perform your testing, deliver your reports and invoice you properly. By using a standard document, we ensure that your information is captured in a timely and accurate way. I've already received a quote. Should I note that in my submission? Yes. If you received a special quotation on a project, please make sure to note the quote number in the area provided on the Contract for Analytical Services. Taking this action will help ensure that your final invoice reflects your quotation. How do I designate the method I want you to use or any special instructions? We ask you to provide the method and its corresponding number (i.e. USP, EP, client method number, AAIPharma method number) you would like our laboratory to perform on your behalf in the space provided on the form. For client or AAIPharma methods, it is important to provide the specific method name or number so we have the necessary information to meet your requirements. For any special instructions for testing or comments, please use the "Special instructions/Comments/Handling" section on the form. What claim/limit do I provide? For finished products, we will need you to note your company specific claims and/or limits. For Raw Materials, please enter the appropriate USP, EP or ACS monograph limits. What if I need a rush service? AAIPharma has built quite a reputation for our ability to handle rush work. Call Client Services at 314-487-6776 or 732-346-5100 to discuss your specific situation to determine if we can deliver the results you expect in the timeframe you need them. I am sending a schedule I or II drug. Do I need a DEA registration and what should I do before sending it? To accept schedule I and II samples, we need a current site-specific DEA registration from the location submitting the controlled substance and a completed 222 Form. Please contact Client Services at 314-487-6776 or 732-346 5100 to request that a 222 Form be sent to you immediately. Sending SamplesWhy would I submit via online submission if I have to send my samples separately? Many AAIPharma clients have requested that the Contract for Analytical Services form be available online for their convenience. This approach provides an electronic way to complete the form and generates a confirmation number that is sent in with physical samples. When your samples arrive at our facility, we match them up with your sample submission form using the confirmation number. How do we know in what condition our samples arrived? If your completed Contract for Analytical Services states that a sample should be frozen or refrigerated, we will contact you immediately if the samples arrive in a condition other than requested. I'm not sure if my sample is a Hazard, what should I do? To ensure the safety of your sample, our employees and the results you expect to receive, please submit the MSDS sheet with your samples for our review. How do I know when my samples arrived? Once your samples arrive at the AAIPharama Services facility, you will receive a confirmation of receipt notification via e-mail or fax. MiscellaneousWhen will my sample get to the lab to start testing? Once the Contract for Analytical Services and samples are received, they are reviewed for accuracy to ensure the sample and contract (lot number, batch number, code number, sample name, time for sampling, etc.) are accurate. The sample is then logged into the system and delivered to the lab. With accurate information on the Contract for Analytical Services, the samples could be sent to the lab on the day of receipt. Without accurate information, the process will be delayed because we do not have the data necessary to meet your requirements. Can you ship me supplies like ice packs, cooler boxes, sterile cups, and TOC vials? We currently do not ship supplies, but if you contact the Administration Manager at 314-487-6776 or 732-346-5100 they can provide vendor contacts and even catalog numbers for the needed supplies. May I use a credit card for payment? Yes. The Contract for Analytical Services provides a space for you to provide credit card information (number, name on card, and expiration). AAIPharma Services accepts MasterCard, Visa and American Express. Can I send the report and invoice to another Company? Yes. We are happy to accommodate requests like this from clients. To do this, we need a signed PO from the third party providing their permission to do so. Additionally, you will need to provide these instructions to us in the "Special instructions/Comments/Handling" section on the form. May I receive the raw data with my report? Yes. We are happy to provide you with the raw data and it is supplied to you at no charge. We just ask you to designate that you want the raw data in the box provided as you complete the Contract for Analytical Services. |
